Patient Admission Forms
We have created our own library of forms for patient admission and patient visits, developed in cooperation with multiple Home Health Agencies, HHA Consultants, and Accreditation surveyors. Its forms cover CDPH, CMS, and Accreditation requirements, and have passed multiple licensure and accreditation surveys. Most forms are required patient forms and are included in the standard forms package, while some are optional and are based on agency needs. With the exception of the “Satisfaction Survey” form, all are NCR/carbonless forms, professionally printed on high quality 8.5×11" NCR stock.

Forms can either be digitally printed with small quantities (10–12) at faster turnaround, or offset-printed with production quantities (100 minimum) at longer turnaround.
Patient Orientation Booklets
Just like our Patient Admission Forms, we have also created our own patient orientation booklet — also known as a patient handbook — to cover CDPH, CMS, and Accreditation requirements. It is customized to each and every individual agency. It is professionally printed on 8.5×11" saddle-stitched stock, with a color cover and B&W interior. Seven specific policies required by regulations are included as general summaries, each referencing directly to the corresponding policy in the agency’s actual Policies & Procedures Manual. Listings of Local Community Resources are matched to the agency's Geographic Service Area.
We will work with you directly during the customization, and arrange reviews of draft copies.
Booklets can be printed in any quantity (as few as one), although cost is higher at lower quantities.
* Total page count depends on the size of your Geographic Service Area, but is typically 52 pages.

Patient Folders
These are 9"×12" presentation folders to hold the patient's admission paperwork: forms, handbook, brochure, business card, etc. While store-bought plain folders can be used, the labor to produce and apply stickers can easily surpass the cost of custom-designed folders.
Agencies can improve the appearance of their admission packets with custom-designed folders. We typically use the design and color scheme of the agency's existing brochure; the color scheme of the agency's logo can also be used for a more generic folder design.
Custom folder printing has a 250-piece order minimum.
We also offer the option of providing a complete folder assembled with forms, booklets, and other print items (all components must be made available).
